What happens if nobody is available to sign for my Self-Query paper response?
Anyone at your address can sign for the package-it does not need to be the individual who submitted the Self-Query. However, if no one is available to sign for your Self-Query response, the United States Postal Service (USPS) will leave a notice with instructions for pickup or redelivery. USPS may attempt delivery up to three times. If the envelope is not claimed within USPS's holding period, it will be returned to the NPDB.
