What if a licensing board is asking for paper Self-Query?
The NPDB encourages all state licensing boards to accept digitally certified Self-Queries. A digitally certified Self-Query response provides assurance that the document is unaltered and displays the exact information that is contained within the NPDB. Digitally certified Self-Queries have faster response times with no extra costs required for mail, and do not require physical processing or storage.
In the event that a state licensing board requires a U.S. Certified First-Class mailed paper copy (signature required upon delivery) of your Self-Query response, you must request a paper copy be mailed to your home or work address. Mailed paper copies of Self-Queries are an additional $13.00. Do not open the envelope before giving it to your licensing or certifying official.
